Scope of Service

This service includes: analysis of microbiome (e.g., 16S stool data); identification of potential patterns or imbalances (dysbiosis); general educational suggestions related to microbiome support and nutraceutical considerations; and a 6–10 week plan of nutraceuticals that may be appropriate for shifting your microbiome.

All outputs are generated through analytical models and interpretive frameworks that are not clinically validated diagnostic tools, may not reflect individual biological variability, and do not guarantee accuracy or outcomes.

Assumption of Risk

You acknowledge that any dietary, supplement, or lifestyle changes carry inherent risks; individual responses may vary, including potential adverse reactions; and you assume full responsibility for any decisions made based on the information provided. No Guarantee of Outcomes

Data Use & Privacy

You consent to submission and analysis of microbiome data, which does not include personally identifiable health information.

Limitation of Liability

You agree to release and hold harmless Alex Zaharakis and affiliated entities from any liability arising from participation in this service.

Client Responsibilities

You agree to follow instructions, avoid overlapping major interventions, and commit to appropriate follow-up.

Non-Disparagement

You agree not to make false or misleading statements about Alex Zaharakis or affiliated entities.

Private Member Agreement

You agree to the PMA disclosed during checkout and purchase of this microbiome consult, outlined at gutbalancing.com/policies/terms-of-service.

Summary of Agreed Terms

By completing this waiver you agree that the data is yours to submit and you consent to Alex Zaharakis and Researched Elements Tech being in possession of the microbiome data. The data holds no patient-specific information — it is strictly a series of letters corresponding to nucleotides mapped for the microbes sequenced and a KIT ID. It will never be used to identify an individual publicly or privately.

I agree, for myself and anyone entitled to act on my behalf, to hold harmless, waive, and release Alex Zaharakis and Researched Elements Tech from any responsibility, liability, demands, or claims of any kind arising from participation in Gut Balancing + Nutraceutical Support. I agree to have read the entirety of this waiver and questionnaire. I acknowledge that this waiver is not medical advice. The analysis and suggestions that follow are not medical advice and should be vetted by a medical professional of my choosing. I agree to consult my medical health practitioner before implementing any microbiome balancing suggestions that may impact my health. I agree that suggestions are for informational purposes only. I acknowledge that Alex Zaharakis is not acting as a medical practitioner and is not providing medical advice. I understand I should check with my practitioner before implementing suggestions, particularly if there are contraindications (e.g., ulcers, GI surgery history, uncommon disorders, etc.). I agree that any suggestions relating to over-the-counter dietary supplements are NOT considered medical advice. I agree not to slander, threaten, produce libel, or make misleading statements against Alex Zaharakis or affiliated entities.
